The problem
As part of a larger project, Hybrid Classroom: Redesign of a New Hire Program, one of the requests was to analyze the effectiveness of the various safety training models within the program and the multiple-hour presentation on Threat and Error Management (TEM) on the last day of the class.
The solution
To alleviate cognitive load for the student, multiple existing safety models and examples were merged into the Human Factors and Threat and Error Management (TEM) models. The TEM model is typically found in only pilot and flight attendant training throughout the aviation industry, but our stakeholders felt it was important to utilize it in the ground operations audience. An introduction module to Human Factors and TEM was developed to be facilitated toward the start of the class, and a TEM interactive activity (one or more) would be embedded into each of the subsequent modules to build on the students understanding. The activities would include real-life internal and external scenarios (e.g., images, voiceovers, videos, etc.) enabling students to analyze the tasks that they were learning about that day, which had the potential of becoming minor to catastrophic incidents.
The process
In line with the larger project, the Analyze, Design, Develop, Implement, and Evaluate (ADDIE) model served as a framework to manage the tasks in SmartSheets.
Met with Project Sponsors, SMEs, and front-line employees to understand the problem

Reviewed current new hire training program, including the various safety models and how they were presented

Visual mockups, outlines, and storyboards for the introduction module and each embedded activities in MS Word
Instructional Design Strategy for the introduction module and embedded activities
Presentation template (PPT) for the embedded activities

These activities included a combination of images, voiceovers, and/or actual videos of incidents. The voiceovers were used to introduce the scenario, and the online tool WellSaid Labs was used. For the videos, they were provided by SMEs and edited in Final Cut Pro
The activity starts with a voiceover naration of what happened during the incident
Then the students are asked what they think were the internal threats (human factors) and external threats that were factors
Next, the students are asked what are the defenses that they think that did not work or were not in place that allowed the incident to happen and if this could have been prevented
Finally, the instructor shares the results of the safety report
The training materials created were included in the larger projects Alpha, Beta, and Train-the-Trainer (TTT) sessions
Alpha, Beta, and TTT sessions included online feedback forms for participants to submit to help enhance the program
Partnered with workgroup training, scheduling, and support systems teams to support the launch of the program
Collaborated with Stakeholders and the internal analysis team to create a plan to evaluate the program in the future by conducting periodic analysis of the completed evaluations from the classroom and OJT, as well as participant and instructor surveys that included items for Human Factors and TEM
At the end of the project, we conducted a review of what worked well and opportunities for improvement
What I learned
For this task, I got an in-depth education in human factors, aviation safety reviews, and how there is never a single reason for what causes incidents but a perfectly aligned combination. This was the first time I experienced using online voiceovers and became very fluent using WellSaid Labs to adjust settings, including timing and pronunciation. It was also an opportunity to learn how to plan and scale an activity in multiple locations within a program.
